Preparing your submission

The application form collects all competitor contact information, your performance repertoire video links, and your program fee in a single entry (all at once). You will need to collect all your information before you submit and have it in hand when completing the form.

All fields are required so preparation ahead of submission is important.

Information you need before you launch the application:

  • Competitor Name – First and Last names of the performing competitor.
  • Date of Birth (Competitor) – Birth date is limited to those 8 to 18 years old by the time of the submission.
  • Age (Competitor) – Age in years at time of submission.
  • Competition Category – Age range in which you choose to participate (Age 8-11, Age 12-15, Age 16-18).
  • Competitor’s Parent or Guardian Name – First and Last name of the responsible party.
  • Email (Competitor’s Parent or Guardian) – Email we will use for communication with the responsible party.
  • Phone (Competitor’s Parent or Guardian) – Phone number of the responsible party.
  • Address (Competitor’s Parent or Guardian) – Home address of the responsible party.
  • Music Teacher’s Name – Competitor’s music teacher.
  • Music Teacher’s Email.
  • Music Teacher’s Phone.
  • Brief Biography (Competitor) – A brief statement of no more than 250 words. You may include relevant musical education, master classes, festivals attended, awards received and/or any public performances. Content will be used for award announcements and program publication.
  • Photo Upload (Competitor) – A photo of the competitor suitable for award announcements and program publication.
  • Preliminary Repertoire URL – A link to your shared PRELIMINARY performance video (see below for more details).
  • Final Repertoire URL – A link to your shared FINAL performance video (see below for more details).
  • Consent – Confirm checkbox to our consent notice and acknowledgement that you have read the rules and regulations.
  • Application Fee – The amount for program participation (not editable).
  • Payment – Credit card payment entry (responsible party).

Saving and sharing your performance videos:

The application form accepts HTML URL links to your shared videos. You will need a place to host and share them so the judges can view your performances. Here are some popular video hosting and sharing options with links for help:
